Latar belakang: Mitti Chikitsa merupakan modalitas pengobatan tradisional yang memanfaatkan unsur tanah (Prithivi/Pertiwi) sebagai sarana terapeutik. Tujuan: Penelitian ini bertujuan mengkaji implementasi terapi tanah dalam sistem pengobatan tradisional dengan fokus khusus pada praktik pengobatan tradisional (Ayurweda dan Usada Bali). Metode: Penelitian menggunakan studi literatur sistematis dengan pendekatan kualitatif deskriptif. Pencarian literatur dilakukan melalui database ilmiah PubMed, Google Scholar, dan ScienceDirect menggunakan kata kunci relevan. Hasil: Mitti Chikitsa telah dipraktikkan lintas peradaban dengan mekanisme kerja meliputi absorpsi toksin, efek termal, kandungan mineral terapeutik, dan aspek psiko-spiritual. Dalam konteks Ayurweda dan Usada Bali, penggunaan tanah terintegrasi dengan konsep Panca Maha Bhuta dan keseimbangan kausa penyakit sekala-niskala. Para pengobat tradisional menggunakan tanah dari tempat sakral yang dikombinasikan dengan ramuan herbal untuk membuat boreh atau pasta terapeutik. Penelitian ilmiah modern mengonfirmasi aktivitas antimikroba unsur tanah terhadap patogen resisten antibiotik, efektivitas anti-inflamasi pada gangguan reumatik, dan kemampuan detoksifikasi. Kesimpulan: integrasi pengetahuan tradisional dengan pendekatan ilmiah modern dapat menghasilkan modalitas terapi komplementer yang aman dan efektif. Dokumentasi sistematis praktik Mitti Chikitsa dalam Usada Bali diperlukan untuk pelestarian warisan pengetahuan tradisional dan pengembangan sistem kesehatan integratif yang mempertimbangkan dimensi bio-psiko-sosio-spiritual.
